The Brazilian forward will move to Real Madrid for €22m and will link up with Real coach Carlo Ancelotti after joining from Juventus.
The move is expected to be finalised in the coming days.
Gervinho had a good season for the Juventus, scoring seven goals in 31 league appearances.
He will become the first player from the south of Brazil to join the Madrid squad and join the club in his homeland.
The 24-year-old scored three goals in 20 Serie A appearances last season and has netted nine times in his 16 La Liga appearances this term.
The player signed a five-year deal with Real Madrid last summer and scored 22 goals in 55 games in all competitions.
